“你nei5係hai6咪mai6痴ci1總zung2掣zai3呀aa3?” means “are you insane?” in English, having a literal meaning of “are you short-circuited?”. You say so to somebody who has done something insane.
For instance, your friend suddenly shout “頂你個肺呀”to your professor in class. You will immediately ask him “你係咪痴總掣呀?” to express your surprise to his weird behaviour.
